He can't explain the spam when he is lucid - that's why it's a mental 
illness and not just a 'bad' choice.  I don't remember all the symptoms of 
schizophrenia, but I have memories of reading that the 'real' person has a 
vague awareness of the irrational behavior, but the behavior is not 
controllable, just as someone who has a muscle spasm or 'tic' can't make it 
stop.  It is not an 'excuse' that your body is not behaving as it should, 
though it is an explanation that apparently he thinks when he is not lucid 
that something alien is implanting the impulse to spam.  In a way it is 
alien since it is not what he, when he is lucid, would do, but whatever bad 
combination of chemcials (or whatever is the cause of schizophrenia) is what 
is causing the behavior.

I don't know about the UK, but, for some reason, in the US, there are many 
people with schizophrenia who are not confined to hospitals, but are 
outpatients.  Many of the homeless have schizophrenia and refuse to be 
treated for it.  I think it is because if they take their medicine, they can 
function.
